What is a Spiritual Care Coordinator?
Tammy provides support to our patients and their families as they address their health needs, and helps them in making decisions about their health care. This may include Advanced Care Planning and Health Care Power of Attorney document assistance.
She provides End of Life care to patients and families, and works with physicians, social services, hospice, and other health care agencies to meet individual needs.
Tammy also assists individuals through the grieving process, here at the hospital, in grief support groups and on the telephone.
Programs/Assistance includes:
Stoke Support Group - for stroke survivors and caregivers, meets every second Tuesday of the month from March through November. (Please note, there will be no Stroke Support Group for July and August, 2010) See Classes & Events.
Mourning Dawns - a five session video-based program that allows you to identify the seasons of your grief and assist you in your grief walk. See Classes & Events.
Advance Dircetives (Healthcare Power of Attorney documents) - assistance with understanding and filling out these documents.
Service of Rememberance - in memory of the loss of your loved one(s), offered in the fall, one time per year. Date provided when available.
