These conversations are part of a process call Advance Care Planning. In this process families are encouraged to have discussions about end of life care and complete a Health Care Power of Attorney document that puts many of those decisions on paper. The usefulness of this document is that during times of crisis we are often unable to think clearly, our memory is blurred and it becomes difficult to make decisions. When there is a document to share with the family the previous conversations become clear again and families are able to honor the wishes of their loved one.
